What is a Primary Care Network, and what do we do?
A Primary Care Network (or PCN) is a group of GP practices working closely together, aligned to other health and social care staff and organisations, providing integrated services to their local population. PCNs add local services to support and complement GP Practices. This enables greater provision of proactive, personalised, coordinated and more integrated health and social care.
The PCN is led by local GPs, who understand the needs of the local patients, wider population, and the General Practice workforce
