Social Prescribers

Our Social Prescribing link workers help patients to get the right support. Many different patients will be referred to Social Prescribers. They include people with long-term conditions, low level mental health issues and those who feel lonely or isolated. Social Prescribers also help people with complex social needs.

Generally, patients speak with a Social Prescriber 6-12 times over a three month period. The Social Prescriber will connect them with activities, groups, and services in their community for practical and emotional support to help them get back on their feet.

Your GP or other clinician may refer you to the Social Prescribers if they believe this would help you. The SPLW would then contact you directly.

At this time, you cannot book yourself in directly, however you can ask your surgery to refer you and they should do this.
